Hospital network: voice intake and scheduling

A HIPAA-aligned intake agent, with a Business Associate Agreement, deployed inside the network's private cloud and integrated with the EHR scheduling system and the payer eligibility gateway.

The challenge

The typical starting point.

A hospital network's central scheduling line often carries long hold times at peak, and abandoned calls pull directly on appointment fill rates. When the phone tree cannot see real-time slot availability, every caller reaches a human, even for a routine reschedule or an insurance re-verification.

Earlier automation attempts commonly stall for two reasons: IVR replacements still need live staff for eligibility and booking, which just moves the queue, and generic voice bots raise concerns about where recordings and transcripts are processed. The network needs automation that respects HIPAA at the infrastructure layer, not just the API layer.

Growth adds pressure. New sites and service lines add call volume without a proportional staffing budget, and leadership wants clear evidence that patient-access pathways are improving.

The approach

How Klivo approaches it.

Klivo deploys a HIPAA-aligned voice intake agent, with a Business Associate Agreement, inside the network's existing private cloud tenant, integrated with EHR scheduling, the payer eligibility gateway and the existing phone system. Recordings, transcripts and model calls stay within the agreed boundary.

Boundary-contained inference

Inference runs in a dedicated private network inside the hospital's cloud environment, so PHI stays within the BAA boundary. Telephony terminates on a SIP trunk inside the same network, including the recording tier.

EHR-native scheduling

The agent reads provider availability, visit types and templates live from the EHR through its FHIR interface. Slots booked by the agent commit the same resources a human scheduler would, so there is no parallel state to reconcile.

Real-time payer eligibility

Insurance is verified through the eligibility gateway during the call, and copay estimates are surfaced before the patient commits to a visit. Self-pay paths hand off to financial counseling with a pre-filled intake form.

Structured clinical handoff

Calls that need a human, such as complex coordination or sensitive specialties, route to a live scheduler with an auto-generated packet: intent, verified identity, insurance status and any flagged sensitivities.

Target outcomes

What this deployment is designed to achieve.

40 to 60%
Shorter time to booking
20 to 35%
Fewer abandoned calls
90%+
Slot-booking accuracy
100%
Handoffs with structured context
What success looks like

Patients book, reschedule and verify coverage in a single call without waiting on hold. Schedulers spend their time on complex coordination, and the compliance team can show that every recording and transcript stays inside the agreed boundary.
